Please note that there is a 4% finance charge added for online purchases. It appears as TAX at check out but please know that it is the finance charge. The web service has not given us the ability to change the term. VENMO OPTION: If you would like to avoid service charge/taxes then you can send money via Venmo to @kbkornhole.

1. Print out THIS FORM and fill in the CORNHOLE REGISTRATION FORM
2. Make checks to SPA PSO
3. Drop form and payment at the SPA front office - in box labeled Cornhole Registration
